1 Fill Out Claim Form

Download your provider’s reimbursement form and fill it out.

2. Attach Itemized Receipt:

You can get it through your user account or from us by sending an email to shopping@allureaid.com

3. Submit Form

Send the form with the itemized receipt to the insurance provider; the address is on the form. You’ll typically be reimbursed within 2-3 weeks.
